We are getting lots of calls re the Supervisor Declaration forms.
The main questions;
1 – Yes you must submit one even if you have done so previosuly.
2 – the most important row is (on the Family form) 4 A (i) “Personal Casework”. If you have over 350 hours of this you can ignore the remainder.
